Population health management brings health concerns into focus and addresses ways resources can be allocated to improve the health of a group of people – a population. The population health approach is proving to have a positive impact at every level, from improving the everyday health of individuals to moving the needle on national spending. At Compwell, we offer the following population health services to meet the needs of and improve the health outcomes of the communities we serve.

Chronic Care Management is a monthly telephonic clinical assessment of a Medicare patient to optimize their health outcomes.
Remote Patient Monitoring is a monthly patient assessment, where a patient’s chronic conditions are monitored with a remote device, such as a blood pressure cuff, glucometer or weight scale.
Annual Wellness Visits are the annual assessments for Medicare patients. This includes colonoscopies, breast exams, etc.

Currently, Chronic Care Management reimburses $60 per patient per month; Remote Patient Monitoring reimburses $100 per patient per month; and Annual Wellness Visits reimburse $124 per year. REMOTE PATIENT MONITORING

Remote Patient Monitoring (RPM) allows health care providers to observe patients with physiological data obtained through a device from anywhere in the world to improve clinical outcomes, manage current conditions, and catch warning signs to prevent issues from becoming problematic. Devices include a blood pressure monitor, glucometer, pulse oximeter, thermometer and weight scale.

Remote monitoring helps patients

RPM enables individuals to take control of their own health and make lasting health improvements while lowing medical expenses. Patients have 24/7 access to medical support through continuous, connected, and focused care.

And providers

RPM allows physicians to extend their reach into patient’s homes to provide clinical care. By receiving insight through vital readings, providers have real-time data to improve outcomes, strengthen patient relationships, and add value to patient care.

How does remote patient monitoring work?

Throughout the day, patients gather information from their device – body weight, blood sugar levels, blood pressure readings, or pulse measurement. Physicians receive this information electronically through a secure, HIPAA-compliant platform. From there, health care providers evaluate the information and take action for their patients, as appropriate.
